How will the vaccination take place at the pharmacy. Facts and controversies

Government, regions, Federfarma and Assofarm have signed a framework agreement that will start vaccinations in pharmacies . Here are the rules and debate among experts (the criticisms of a student of Prof. Ricciardi)

Covid-19 vaccinations can start at the pharmacy.

The government, the regions, Federfarma and Assofarm have signed a framework agreement which, as required by the Sostegni decree, will start vaccinations in pharmacies.

Excluding the possibility of vaccinating subjects with "extreme vulnerability" or with a positive history of a previous severe allergic / anaphylactic reaction and the priority criteria indicated by the government must be followed.

The presence of nurses is not foreseen. On this doctors and experts express doubts. All the details.

THE AGREEMENT

With the government protocol, the regions, Federfarma and Assofarm intend to “start the administration of the anti SARS CoV-2 vaccines by the affiliated pharmacies pursuant to article l, paragraph 471, of law no. 178 (Budget Law 2021), as replaced by art. 20, paragraph 2, lett. h), of the decree-law of 22 March 2021, n. 41 (Legislative Decree "Sostegni") ". All the participating pharmacies, on a voluntary basis, "must notify according to the terms and conditions set out in Annex I to this Agreement".

6.00 EURO PER INOCULATION

For this service, the pharmacies will be "paid a remuneration of € 6.00 (six / 00) for the professional act of the single vaccination inoculation".

THE RESERVATION

Pharmacies will also have to respect the priority criteria dictated by the Government's planning. "The booking and execution of vaccines – the protocol reads – will be carried out by the pharmacies, according to the programs for identifying the target population previously defined by the competent health authorities and following the related priority criteria".

NO TO VULNERABLE SUBJECTS

In pharmacies, the possibility of administering the anti Covid-19 vaccine is excluded "towards extremely vulnerable subjects or those with a history of a previous severe allergic / anaphylactic reaction".

"The administration of the vaccine in the pharmacy will take place only after obtaining informed consent and the relative anamnestic form for the evaluation of the suitability / inability of the subject to undergo the vaccine administration", states the protocol.

ADVERSE REACTIONS

Also according to the protocol, after vaccination, the patient must stay in the pharmacy monitoring area for 15 minutes. The pharmacist will supervise and provide emergency support in the event of an anaphylactic reaction due to vaccination, alerting 118. The emergency room should also be called in the event of urticaria, respiratory or haemodynamic problem.

A SPECIAL TRAINING COURSE

Pharmacists who will have to carry out the inoculation are "required to attend the ISS ID I 74F20 course" Covid-I9 vaccination campaign: the safe administration of the anti SARS-CoV-2 / Covid-19 vaccine ", which will be extended beyond the date expiration date foreseen and implemented with a specific module in FAR relating to specific skills of the pharmacist, also with regard to the vaccine inoculation activity and the compilation and relative interpretation of the standard pre-vaccination triage form, prepared by the Istituto Superiore di Sanità in rule of art. I. paragraph 465, of the 2021 Budget Law ".

“Successful completion of the aforementioned course enables them to carry out vaccinations in pharmacies. Pharmacists also undertake to attend updates to the aforementioned modules that may be prepared and made available in relation to the implementation of the vaccination campaign and the new authorized vaccines. The aforementioned course must also be attended by pharmacists who have successfully completed the ID 145 course n. 301217 of UTIFAR, to be considered as an introductory element to the topic of vaccination in pharmacies ”, reads again in the framework agreement.

THE POLEMICS OF THE FEDERATION OF THE ORDERS OF DOCTORS

But no training, for professionals, can ever replace the skills and experience of doctors. Fnomceo, the Federation of Orders of Doctors and Dentists, argues that a medical presence is also required in pharmacies. "We understand and share the government's need to increase the points of administration of vaccines, when we finally have to have them in sufficient quantities", said the president, Filippo Anelli, adding that "however, the issue of safety remains essential. . The vaccine is a drug and must be administered, as required by regulatory agencies, only with a doctor's prescription, i.e. after anamnestic and clinical evaluation. That is, always under the supervision, in the presence, of a doctor, who can collect informed consent, assess the patient's state of health and promptly manage any side effects. This is provided by the Italian Medicines Agency, which expressly recommends in case of vaccine administration "urgent medical assistance if symptoms of a severe allergic reaction occur".

CAROLINA CASTAGNA: WE NEED AT LEAST NURSING STAFF

Carolina Castagna , doctor specializing in Hygiene and Preventive Medicine at the Agostino Gemelli IRCCS University Hospital Foundation , Catholic University of the Sacred Heart, and student of Professor Walter Ricciardi (advisor to the Minister of Health, Roberto Speranza), considers the presence of specialized personnel necessary : “I find it a bit reckless not to foresee the presence of medical and nursing staff”, said Castagna, speaking to Otto e mezzo, La7 program.

“I believe that vaccination is a medical act and must be done in total safety, with the right precautions and the right skills. I am happy with the agreement signed with the pharmacies, but I am rash at not foreseeing the presence of medical, nursing and emergency personnel ”, he added yesterday during the broadcast conducted by Lilli Gruber.

MATTIONI: EMERGENCY STAFF NEEDED

Guido Mattioni, journalist and writer, co-author of various books with the oncologist Maria Rosa Di Fazio, also shares the same opinion, who writes in a post on Facebook : "I quote as this agreement states:" In case of anaphylactic reaction due to vaccination will be notified immediately 118, whose intervention will be ensured as soon as possible ". The same will be done in case of "sudden urticaria, respiratory or haemodynamic problems". It would be funny if there were no crying. This agreement therefore presumes to know already today the availability and timing of intervention of ambulances – traffic and weather conditions included, day by day and for the months to come – throughout Italy. Even the conditions for reaching a pharmacy in an isolated location. This is when in case of anaphylactic shock the timing of intervention – moreover those of a Real Doctor, one who perhaps has experience of PS and emergency medicine – are played in a few minutes. Do we want to entrust this to the pharmacists? It is a simple question. According to the rules of the Italian Society of Allergology, the first phases in case of anaphylactic shock foresee in fact three phases to be performed without wasting a minute and above all coldly, professionally, therefore by a doctor – and not even any one, but also with proven experience – and certainly not therefore of a very respectable pharmacist who does another job 365 days a year ”.

GENOA: VACCINATIONS BEGIN

Apart from controversy, Genoa has already started vaccinations in pharmacies. At the moment there are 52 businesses involved which, when fully operational, will be able to vaccinate 400 people a day between the age groups 70-79 years.

“Genoa is the first in Italy to administer vaccines in pharmacies. An important example, a big step forward for the whole of Italy ". The vaccination campaign currently involves 52 pharmacies and, when fully operational, it will be able to involve about 400 people per day aged 70-79, but not the so-called 'ultra-frail' subjects ”, commented the mayor of Genoa, Marco Bucci.
